This assessment consists of:

 

• A 1 hour meeting with all concerned parties to identify the history and challenges facing the family with an Occupational Therapist

• A medical assessment to identify health history with our Nurse practitioner.

• A detailed report summary and action plan.

• A second one-hour advocate session to discuss the plan and next steps
